Access to Medical Records

In accordance with Current Data Protection Legislation, patients may request to see their medical records. These requests are known as Subject Access Requests, and a request can be made verbally or in writing, however we always seek to fully identify patients, and obtain the relevant authority to release information, unless we are legally obliged by the authorities to do so. Completing the attached form will help us provide you with what you require quickly.

Due to the introduction of GDPR we have made the decision to no longer allow online access for under 16 year olds. We are very sorry if this comes as a disappointment to you but we pride ourselves on maintaining effective access to confidential medical records for all our patients and decisions like this are only made when we feel it is absolutely necessary to safeguard patient information. We will therefore be switching this service off on 11th July 2018. We will also be stopping any parental or guardian proxy access to their child’s record if they are aged between 16 and 18.

We will review this policy periodically as our clinical system is updated. Currently there is no facility that enables us to undertake the constant evaluation required by GDPR to provide this service safely.

We are aware that you may use this service to order medication for your child and so we would like to let you know that you can still do this electronically by using our repeat prescription form on this website.
